Appendix 4000-2: Supervisor and Secondary Approver Time Frames

This appendix states the required time frames for a supervisor and secondary approver to complete various actions in an investigation.

Approve or reject a safety plan	Before implementation of the safety plan, if the plan involves any of the following: Restricting a person’s contact with children. Restricting a person from supervising other caregivers. Temporarily restricting an operation from operating. No later than 48 hours after implementation of the safety plan, otherwise.	N/A
Approve or reject an extension request	Within 5 days after the request’s submission and either of the following: No later than 30 days from the date of the intake report. No later than the calendar date on which an extension expires.	N/A
Do either of the following: Approve the investigation in IMPACT. Reject the investigation for significant documentation errors or incomplete investigation activities.	Five days after the investigation is submitted to the supervisor for approval.	Five days after the supervisor submits the investigation to the secondary approver for approval.
Approve or reject an investigation that has been resubmitted after being rejected	Five days after the investigation is resubmitted to the supervisor for approval.	Five days after the investigation is resubmitted to the secondary approver for approval.
