Religious record of birth recorded in the U.S. or its territories

A Baptismal Certificate Showing U.S. Birth must be recorded within 3 months of birth, and indicate a U.S. place of birth showing either the date of birth or the individual’s age at the time the record was made.

Documents that establish a U.S. place of birth or in some way indicate U.S. citizenship are acceptable. Below are examples of such documents:

Use IMPACT option – U.S. Birth Certificate

Statement showing a U.S. place of birth

The statement must be signed by the physician or midwife who was in attendance at the time of birth.

Use IMPACT option – Record of Birth in U.S. Hospital

Affidavit from two adults regardless of blood relationship to the individual

The caseworker must contact a regional attorney to determine whether this option can be used in the case-specific situation.

Affidavits are only to be used as a last resort if the individual is unable to provide any other documentary evidence of citizenship.

Seneca Indian Tribal census record

None.

Use IMPACT option – Citizenship Certificate

Bureau of Indian Affairs tribal census records of the Navajo Indians

None.

Note: If none of the documents above exists and the child is under the age of 5, was found in U.S., and the parents are unknown, staff must contact the DFPS regional attorney to determine whether the under age 5 option may be used in the case-specific situation. Use IMPACT option – Child under age 5 found in U.S., parents unknown.
